When a life-limiting incurable illness (life expectancy of six months or less) no longer responds to treatment, or when treatment severely diminishes the patient’s quality of life and the patient or their responsible party elects for no further aggressive treatment, it is time to consider hospice care. Once the patient is enrolled in hospice care with Anchor Health, our team (which includes Hospice Medical Director access) is available to call, consult with, triage, and/or visit as needed 24/7, 365 days a year. The hospice team will coordinate for a personalized visit program designed specifically for addressing the patient’s care needs on a regularly scheduled basis.
